The clinical friction

Having the equipment is only part of being ready.

A team may have every item it needs and still spend time finding the right size, arranging equipment or checking what has already been prepared. When each clinician sets up differently, the person assisting has to learn that layout while also following the airway plan. Adult SCRAM™ addresses this preparation work by giving it a consistent physical structure.

The system

A shared layout for the airway plan and its rescue options.

SCRAM™ stands for Structured CRitical Airway Management. Adult SCRAM™ brings that approach into a portable system with a primary working field, a separate lid filing system and a removable surgical-airway panel.

The bag opens into a kit dump: an arranged working surface where the team can lay out and check equipment together. Keeping the primary equipment and reserve items in distinct areas allows the team to prepare for a change of plan without filling the working surface with everything it carries.

How the layout supports preparation.

The three working areas have different jobs. Their arrangement is intended to make preparation easier to share and check, while keeping rescue equipment accessible if the clinical plan changes.

The primary working field

The high-contrast primary field places large-adult preparation on the left, small-adult preparation on the right and shared equipment through the centre.

This gives the person preparing and the person assisting the same reference points. They can see where equipment belongs and check the setup together before the procedure starts.

Design move 2

Secondary filing system and rescue landmarks

The grey filing system in the lid holds tubes, rescue devices, basic airway adjuncts and the surgical-airway pouch. These remain accessible without occupying the primary working field.

The filing tabs identify specific equipment: yellow marks the surgical-airway kit and blue marks the thoracostomy kit. The colours help the team locate the relevant section of the bag.

A separate surgical-airway working field

A weighted pull-out panel can overlay the primary kit dump or detach to form a second working field. It gives surgical-airway equipment its own preparation area.

The panel has two labelled sides: yellow for the Physician / MERT scalpel–finger–bougie configuration, and blue for the CMT surgical-airway kit. These are the panel configurations; the coloured filing tabs in the lid identify stored equipment.

The details that support everyday use.

A preparation system has to remain usable between cases. Teams need to know what belongs in it, how it is checked and how it is restored after use. Storage zones, a kit list and visible prompts give those routines a common starting point.

Local teams still determine their equipment, medication arrangements and clinical protocols. The design provides a layout around which those decisions can be taught, checked and maintained.

Structured storage zones

Dedicated areas support a reproducible layout for laryngoscopes, rescue devices, syringes, filters, airway adjuncts and procedural equipment.

Stencilled kit dump

A stencilled working layout supports efficient pre-stocking, visual checking and repeatable setup before the procedure is required.

Pull-out surgical airway kit dump

The panel can be placed over the primary kit dump or detached for a separately prepared surgical-airway field. Its weighting is intended to limit movement in exposed working conditions.

Grey filing system (bag lid)

The lid separates additional airway equipment from the primary field. Yellow and blue filing tabs identify the surgical-airway and thoracostomy kits respectively.

High-contrast indicators

Contrasting backgrounds and equipment landmarks make the working areas easier to distinguish.

Bougie storage

Dedicated bougie storage supports rapid access and protects equipment from being buried inside the bag.

Drug loops

Integrated loops support secure, visible preparation of emergency medication where local policy and governance allow.

Checklist pouch

A high-visibility pouch supports RSI checklists, local protocols or crew prompts without adding clutter to the working surface.

Barcode, RFID and NFC options

Identification options can support local stock control, audit and equipment management systems.

Lockable zips

Lockable zip sliders support tamper-evident systems where required by local governance.

Wipe-clean material

Adult SCRAM™ is made from wipe-clean, durable material designed for operational environments where portability, cleaning and repeated use matter.

Designed for pressure

The layout has to work when attention narrows.

When the airway plan changes, the team still needs to communicate, locate equipment and prepare the next working area. Adult SCRAM™ is designed to reduce how much of that organisation must be improvised during the procedure, with reserve equipment and a separate surgical-airway panel prepared as part of the initial setup.
